To enroll your child:

Application forms are in PDF [Portable Document Format] and require the Adobe Acrobat Reader plug-in.   Click here to download this free software.

1. Please complete an application form in English or en español and submit it by mail or in person to the appropriate school.
2. We also need copies of the following documents:
   • birth certificate
   • proof of address (a utility, mortgage or insurance bill)
   • your child´s immunization records
